Major hazard barriers reviewed periodically — never seen live.

Bow-tie analysis is conducted on a periodic cycle. Safety Critical Element integrity is documented in static records but never visualised continuously. Shift managers and process safety leaders have no real-time view of which barriers are Intact, Degraded, Absent, or Unassessed across the bow-ties relevant to current operations. Barrier degradation is identified after incident, not before.

02 · Isolation

Multiple isolation systems with no unified live view.

Major facilities typically operate several isolation registers in parallel — electronic permits-to-work, mechanical lock-out tag-out, valve registers, blind/spade lists, electrical isolation certificates, high-voltage controls, and decontamination tags. No single screen shows the complete isolation picture. Cross-system conflict detection is manual and ad hoc.

Shift handover reports are unstructured narratives. Quality depends on the writing skill and time available of the outgoing shift. Institutional memory leaks at every shift change. The founder is a former operator and process safety engineer with twenty-eight years of major hazard operational experience at one of the world's largest LNG facilities. He progressed from the field to senior operational leadership, serving as Shift Manager (the senior shift authority accountable for safe production across multiple LNG trains) and in process safety and operations coordination roles — process owner for the permit-to-work system, lock-out tag-out programme, isolation registers, and operations competence assurance.

He held Technical Authority for both Operations and Process Safety. The platform is designed by someone who held the roles it supports — every product decision grounded in operational vocabulary, workflow understanding, and regulatory context that comes from having lived inside major hazard operations for nearly three decades.
